Life Insurance

A temporary waiver of contributions for Optional Life (OLI) and Dependent Life (DLI) for GM hourly employees and retirees will be implemented. 

·        Employee contributions for OLI and DLI will not be collected from pay for the period February through May 2007. 

·        Retiree and surviving spouse contributions will not be collected for the months of March through June 2007 for OLI and DLI coverage.

 

Did you know you are eligible for Basic Life, Extra Accident, Optional Life, Dependent Life, and Personal Accident Insurance on the first day of the month following the month in which your employment commences?  The life insurance provided, as well as, costs for elected policies varies with each individual.  Optional and Dependent Life Insurance rates significantly increase every five years, beware of this.  Contact your local Benefit Representative or call Fidelity at 1(800) 489-4646 for clarification of benefits.

 

Do you know who your beneficiary on file is?

It is a good idea to update beneficiary designation every three years or after a family death, birth, or divorce.  Use www.gmbenefits.com to update or call 1(800) 489-4646 for a designation form.

 

Death Reporting Process

In the event of a death, whether an active employee, retiree, or dependent it is important to notify your Local Benefits

 

Office when the death occurs.  You or your family may do so by calling 246-3450 or 1(800) 730-8292.  If the deceased was an active worker or retiree, your Local Benefit Representative will ask the caller information such as:

·        Name of deceased

·        Date of birth

·        Date of death

·        Visitation

·        Location

·        Location for funeral services

·        Memorial contributions

·        Source of caller

 

The Benefit Representative will then notify Personnel and the Local Union Hall, and have a posting on Auto View for the GM family.  The caller will then be advised to call Fidelity at 1(800) 489-4646. 

 

After receiving the reported information, the GMBSC will then inform all units necessary and send a packet of information to the address of record that includes information on health care, pension, life insurance, and PSP.

 

Legal Services

Is your house in order?  We don’t like to think about death, but it’s going to happen!  Make it easier on your family and have a will prepared.  GM Legal Services will prepare your will, a power of attorney, and a living will at no cost to you.  Consider giving them a call at 531-7722.  Use your benefits!
